Pain on the outside of the hip is often caused by gluteal tendinopathy, not the hip joint itself. The gluteus medius and minimus stabilize the pelvis during walking, running, and single-leg activities. Tendinopathy develops when these tendons break down due to overload, poor recovery, or reduced repair capacity.
Common causes include hip abductor weakness, poor pelvic control, sudden increases in load (hill running or side sleeping), and low estrogen levels.
